Clinical Preparedness Checklist
Step 1: About 2 Months Before Placement

Confirm your immunization records.

If records are missing, you may need bloodwork (titres) and possibly new shots (can take 2 months).

Ensure doctor completes and signs all shaded areas + stamps the Health Care Provider page.

Collect and attach:

Hep B, MMR, Varicella titres
-
Chest X-ray + doctor’s note (if TB test is positive)
-

Required before working with patients.
-
Apply through local police department.
-
Must be valid for the whole placement.
-
Renewal: Yearly (PND, 320, 420), every 6 months (220 & 520 students).

Lab proof of immunity (titres):

Measles, Mumps, Rubella (MMR)
-
Varicella (Chickenpox)
-
Hepatitis B
-
-
Doctor’s proof of immunizations:
-
Tetanus, Diphtheria, Pertussis

Polio
-
Flu shot
-
-
Renewal: Updated yearly and valid for placement.

Required: Baseline two-step test (done once).
-
If negative → one-step yearly.
-
If positive → chest X-ray every 2 years + yearly doctor’s note saying "you’re free of signs and symptoms of active TB"
-
Include in form: Chest X-ray report + doctor’s note if positive.

Strongly recommended yearly (October–November).

If you don’t get one, you may be removed from placement during an outbreak.

Step 2: About 1 Month Before Placement

Register for a 2-day course.

Renewal:

First Aid = every 3 years

CPR = yearly
