Clearance Review for Seneca OTA/PTA Students
Clinical Preparedness Checklist
Step 1: About 2 Months Before Placement

Bring your Medical Form for signatures & stamp.

Confirm immunization records. If missing, you may need bloodwork (titres) & new shots (can take 2 months).

Collect and include:

Hep B, MMR, Varicella titres

Chest X-ray + doctor’s note if TB test is positive

Renewal: Before each term if anything expired
-

Apply for Co-op Work Permit → Application Form
-
Upload proof of application for review
-
Once permit arrives, book clearance review + upload to Concise Health Solutions
-

Lab proof (titres): MMR, Varicella, Hep B
-
Doctor’s proof: Tetanus, Diphtheria, Pertussis, Polio, Flu shot, COVID

Renewal: Must be current & valid for placement

Required: Baseline two-step test (done once).
-
If negative → one-step yearly.

If positive → chest X-ray every 2 years + yearly doctor’s note saying "you’re free of signs and symptoms of active TB"

Include in form: Chest X-ray report + doctor’s note if positive.

Get in Oct–Nov each year.

Required to stay in placement during outbreaks.

Step 2: 1 Month Before Placement

Complete 2-day course.

Options: Concise Health Solutions or WSIB-approved providers.

Renewal: First Aid = every 3 yrs, BLS = yearly

Worker Health & Safety Awareness (4 Steps)

WHMIS (80%+ required)

Renewal: Once, before first placement

Download + sign: 2024 Authorization to Collect and Disclose Personal Information

Renewal: Once, before first placement

Book at Concise Health Solutions or other providers:

Renewal: Every 2 yrs

Bring completed Medical Form, VSS, First Aid/CPR, Mask Fit.

Renewal: Before each placement if any documents have been updated.
