Clinical Documentation Checklist

Step 1: About 2 Months Before Placement

    • Bring your Medical Form for signatures and stamp.

    • Confirm your immunization history. If missing, you may need bloodwork (titres) and new shots (can take up to 2 months).

    • Collect and attach:

      • Hep B, MMR, Varicella titres

      • Chest X-ray + doctor’s note if TB test is positive

    • Renewal: Before each term if anything has expired

    • Apply online through your local police department.

    • Must be valid for the entire placement.

    • Renewal: Every year

    • Must include proof of:

      • Hepatitis B (lab titres)

      • Tetanus, Diphtheria, Pertussis

      • Measles, Mumps, Rubella (MMR)

      • Varicella (Chickenpox)

      • Polio

      • Flu shot

    • Renewal: Every year

    • Required: Baseline two-step test (done once).

    • If negative → one-step yearly.

    • If positive → chest X-ray every 2 years + yearly doctor’s note saying "you’re free of signs and symptoms of active TB"

    • Include in form: Chest X-ray report + doctor’s note if positive.

    • Get in October–November each year.

    • If you don’t get it, you may be removed from placement during an outbreak.

Step 2: 1 Month Before Placement

    • Book with Concise Health Solutions (or another provider).

    • Renewal: Every 2 years

Step 3: Once All Documents Are Ready

    • Bring your completed CPP form and documents.

    • When to book:

      • November → January placement

      • March → May placement

      • July → September placement

    • Renewal: Before each placement if any documents are updated
